Registering with the Practice Online
We are currently open to new registrations if you live in our catchment area (click here for map).
To complete a patient registration online please complete the steps shown below for each person being registered:
- New Patient Questionnaire
- New Patient Registration Form
- Once submitted please email 2 forms of identification, 1 Picture ID such as Drving Licence or Passport and 1 proof of address such as a Utility Bill or Council Tax Bill. These can be emailed to the following address Fleetmedicalcentre@nhs.net If you have just moved into a new property and do not have proof of address please advise when submitting your picture ID
Please note unless you complete all of the steps above we will not be able to register you.
We will only hold partial paperwork for 3 months, if you do not complete after this date these will be destroyed and you will need to repeat the full process again.
Hard copy registration forms are available from reception and large print forms available on request.
If you have come to this country from abroad, then it is necessary for you to provide photocopies of paperwork confirming your entitlement to remain in this country for more than 6 months. Original documents should also be available for us to view and photocopies can be made by the Practice.
Examples of suitable documents:
- A current British or European Community Passport
- A work permit
- A valid visa (with more than 6 months left to run)
- Home Office paperwork for asylum seekers or refugees
- If you are a student, a letter from your college/university confirming the duration of your studies
You will be given a named doctor who will be responsible for your care. Where possible you should see or speak with this doctor for on-going issues. However, you may speak or see other doctors for more urgent/new problems.
Adults will be asked to attend a new patient check with our Health Care Assistant to collect baseline measurements and assess lifestyle risk factors.
If you take regular medication then please make an appointment, by telephone or at surgery, to discuss with your doctor before a repeat prescription is due.

Temporary Patient Registrations
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
